The following forms may be downloaded and submitted to the Rockaway Township Police Department online: Traffic Bureau Traffic Enforment Form - The Rockaway Township Police Department actively responds to complaints of ongoing speeding problems and other traffic infractions that occur within the Township.  If you beleive that there is a speeding or other traffic related issues in your neighborhood, please complete this form and return it to the Traffic Bureau. The following forms may be downloaded and must be submitted to the Rockaway Township Police Department in-person.  Some forms and applications may require a fee. Firearms: *SBI-212A Request for Criminal History Record Information This form must be submitted to the State Police using the original yellow form. For this reason, we are unable to make it available for download and must be picked up at the Rockaway Township Police Department Detective Bureau. This form is required for any change in firearms application, renewals, or all handgun purchases. Mental Health Consent - This consent MUST be completed by the firearm application.  Faillure to consent requires denail or disapproval of the application. Voluntary Form of Firearms Registration - This form may only be utilized to voluntarily register firearms you own that may be lawfully possessed in New Jersey that were previously acquired.  You may not utilize this form to acquire firearms. Certificate of Eligibility - This form must be used in the transfer of a firearm. Instructions for Firearms Purchaser Identifcation Card (New or Renewal) Application for Firearms Purchaser Identification Card - This form is prescribed by the Superintendent for use by applicants for Firearms Purchaser I.D. Cards & Handgun Purchase Permits.   (Form STS-033) Application for Permit to carry a handgun - Application must be delivered, in triplicate, to the Chief of Police of the municipality wherein you reside, or to the Superintendent of State Police in all other cases.
